How to set up my Questionnaire?

NOTE: If you are wanting to learn about the Questionnaire feature of your software, go to Support > Getting Started.  Details on how it is useful to you are contained in the Client Management CRM section of Getting Started.

To setup your Questionnaire feature, you will need to do the following:

BASIC SETTINGS

  1. Go to menu item MEDIA & LAYOUT > My Form Designer
  2. Select the Questionnaire Form Tab
  3. Decide whether you would like to have the questionnaire send automatically, or manually
    1. AUTOMATICALLY ON FIRST BOOKING: Select YES
    2. MANUALLY: Select NO (this is the default).  If you set this to NO, you can then manually send the Questionnaire to a client within that client’s CRM page.
  4. If you want to change the name of your Questionnaire (perhaps Pre-Appointment Form etc), you can do so here.
  5. Click Save Changes

EMAIL THAT SENDS THE QUESTIONNAIRE

  1. If you are still on the above page, click the button “EDIT THE EMAIL TEMPLATE”
  2. On this page you will see three text boxes:
    1. SUBJECT
    2. EMAIL BODY (PART 1)
    3. EMAIL BODY (PART 2)
  3. We have provided a basic template for your email content.  You may change it here to suit your needs.  Note there are two placeholders you can use in your email:
    1. {{first_name}}
    2. {{last_name}}
  4. NOTE: Please ensure you use the placeholders EXACTLY how they are written, for example {{first_name}} will ensure that the client’s first name is outputted in your email when it is sent.  As an example, “Dear {{first_name}}” will return “Dear Shannon” or “Dear Jim” etc depending on your client’s actual name.
  5. The EMAIL BODY (PART 1) and (PART 2) are the two parts of your email.  Between these two parts is a link to your Questionnaire the client can click to complete the Questionnaire.  The link is auto generated, all you have to do is write the email you want to send your clients.
  6. So, as an example, your email could look like this (First part before the CLICK TO COMPLETE… link.  Second part after that autogenerated link):Dear {{first_name}}

    Thank you for booking an appointment with APPLE BUSINESS COACHING.

    Prior to your appointment I would request that you complete the following questionnaire. Simply click on the following link:

    CLICK HERE TO COMPLETE THE QUESTIONNAIRE

    This questionnaire will help me gain a better understanding of why you are coming to see me.

  7. Click Save Email Template

CONTENT OF YOUR QUESTIONNAIRE FORM

  1. The last part of setting up your questionnaire is the content of the questionnaire form itself.
  2. Go to menu item MEDIA & LAYOUTS > My Form Designer.
  3. Select the Questionnaire Form tab
  4. Click the button EDIT CONTENT OF QUESTIONNAIRE HERE
  5. On this page you will find our drag and drop form editor.  It is an easy to use form creator.  We have already created a basic template form for you.  You can further customise this, or remove elements you do not want and rebuild it entirely if you wish.
  6. Essentially, all you have to do is drag and drop the elements you want onto the form in the location you want.  You can then change:
    1. Label of the element (for example: Your Age)
    2. The Values of the element (for example, you might want a drop down and the values you give might be (1) 0-18 (2) 19-34 (3) 35-50 (4) 51-onwards).
    3. Whether it is a required field (compulsory or not)
    4. Whether you want placeholder text (for example, a text box might say “Please tell us why you are coming to see us” as a friendly prompt to your client
    5. And many other advanced features if you wish.
  7. If you want to move the location of any one element (i.e. place an element higher or lower on your form) you can simply highlight over the element on your form, locate the four pointed arrow symbol and drag the element to its new location
  8. NOTE:  To find detailed information on how to use our drag and drop form editor, please go to our Support site and go to the Client Management – CRM section and locate the subsection on Forms
  9. Once you are happy with the content of your Questionnaire, click Update.

Your questionnaire form is now set up and ready to use.
